Some forms have been prepared to allow customers to fill in the blanks with their personal computers. The forms are in the PDF format and you will need the most recent version of Adobe Acrobat Reader installed on your personal computer to enter data, view and print the forms. Acrobat Reader is available free from Adobe's official Website. To download the software, please visit the Adobe Web site and follow the instructions provided.

  Use your computer to fill out forms:
Download the form that you want to use. Place your cursor on a blank field, click and begin to enter your data into the field. You can tab to the next field until the form is completed. If you are using the free Acrobat Reader, you cannot save your data so you will need to print a copy for your records and one to mail.
(Hint: Most check boxes can be checked by clicking on the box or hitting the space bar.)
  To print one of the documents:
  • Click on the printer icon located at the top left region of the Adobe Acrobat toolbar; or from "File" pull-down menu, select "Print" option.
    [The "Print" dialog box should appear at this stage]
  • Select the "Shrink to Fit" option in the dialog box. Some printers crop the text of the forms at the edges; the "Shrink to Fit" option prevents this from happening.
  To print an 8 1/2" x 14" document:
  • If you are attempting to print an 8 1/2" x 14" document, click "Setup" button with your mouse.
    [The "Setup" dialog box should appear at this stage]
  • Select 8 1/2" x 14" from "paper size" options.
  • Click "OK" button.
    [This should return you to the "Print" dialog box]
  From the "Print" dialog box:
  • From the "Print" dialog box, click "OK" button.
    [This will send the document to the printer]
  To download one of the documents:
  • Select "Save" icon on the Adobe Acrobat display.
    [This will open a "Save As" dialog box]
  • Select the disk drive and directory, if applicable, onto which you would like to save the document file
  • Click "Save" button.
    [A "Copying file" bar which displays the status of the file transfer will appear]
